- Is bariatric surgery covered on my plan?
- Is bariatric surgery performed at CHRISTUS St. Vincent covered on my plan?
- What are the criteria I must meet for approval.
- What is my financial responsibility (deductible/copays)?
Finding out this information in advance will significantly speed up the paperwork and approval process.
